Alzheimer’s & Dementia Care

Caring for a loved one suffering from dementia can take a huge toll on a family. It’s difficult and it’s depressing. As Alzheimer’s Disease progresses, communication becomes more difficult, and as patients lose memory and motor skills, there are many physical as well as emotional challenges.

For many families, the goal is to keep their loved one in their own home, in familiar surroundings. Prestige Healthcare Resources provides the compassionate care Alzheimer’s and Dementia patients need, and also meet the needs of the family.

Our goal-based care plans are specifically tailored to your needs, and will evolve as those needs change.


Who will make healthcare and/or financial decisions when the person is no longer able to do so?
If your loved one is still lucid enough, getting their wishes down on paper means they’ll be preserved and respected by all members of the family. There should be a power of attorney, both for finances and for healthcare. If your loved one has already significantly declined, you may need to apply for guardianship/conservatorship. An elder care attorney can assist you in this process.

Where will they live?
You’ll need to determine if their own home is still suitable to their physical and mental difficulties, both currently and as their needs change. Sometimes homes can be adapted. If they live alone, they will require more care. Is relocating to live with a family member a viable option? For many patients a facility may eventually be the best option.

How will all their care needs be met?
Caregiving is a huge commitment and huge challenge. Eventually, an Alzheimer’s patient might require round-the-clock care. Family members can only do so much, especially when there are medical needs. And as much as we would like to always be there for our loved one, we have our own commitments, such as jobs and the needs of our own families.

Transition Care


Whether it’s the result of surgery, stroke, or any medical issue, transitioning from a hospital or rehab facility to life back home can be challenging both for the patient and their family. In-home care is often needed, sometimes for a lengthy period of time.

When someone is recovering from surgery, they often have bandages that need changing, stitches or staples to monitor, are uncomfortable or in pain, and have limited mobility or lack the ability to perform normal daily routines.


From nursing and other medical care to therapy, rehabilitation services, and whatever other care is needed, Prestige offers a complete care package.

Our transition team will assist you every step of the way. It starts before you leave the hospital or other facility. We have a designated Liaison staff that meets with the patient or their family, collaborates with the discharge planner or social worker, and provides a free comprehensive home assessment.

Once home, our nurses and other staff will assist with pain management, dressing and bathing, physical therapy, and if necessary, cooking and other household responsibilities.


When a stroke patient returns home, their home environment can greatly impact their recovery. From more severe effects such as paralysis and memory loss to weakness, lack of balance or coordination, or numerous other problems, a stroke can change the way a patient functions in their home.

Prestige provides a free home modification assessment, and we will put together a care plan that meets all the needs of the patient and the family. Whatever your needs, and whatever type of facility you’re coming home from, Prestige will make your transition as easy as possible.


Hospice Care

Choosing Hospice care for a loved one is one of the most difficult and gut-wrenching decisions a family can make. End-of-life care is not about how a person dies, it’s about how they live. They deserve compassion, comfort, and dignity.

Home care does not have to just be in the home. Unlike many other home care providers, Prestige Home & Community Support is a full-service provider.

We will even accompany them on a trip or vacation. When people think about in-home care, they don’t always think about leaving home. But these things can be especially important for that independent lifestyle, which is why they’re part of our full-service Concierge Care.

Many senior citizens no longer drive. That is a loss of independence, and it can be a huge cost and inconvenience. Cabs are costly, shuttle buses are difficult, and even if adult children live nearby, they have jobs and other commitments. Our Concierge Care includes personal transportation from someone they know and trust. From doctor’s appointments to the supermarket or any errand.

Michele has served as a clinical nurse with Prestige Healthcare Resources for over 7 years. She has worked as a hospice nurse in both inpatient and home care settings, a clinic nurse providing patient education and medication administration/management, a med-surgical nurse in the hospital setting, and as a home care nurse.
TRACY STATONManager & Admin
Tracy serves as the Manager, Homecare Division and as the Administrator, Home Support Division of Prestige Home and Community Support. She has a degree in Gerontology and Aging Services and have worked in the healthcare field in many arenas for the past 28 yrs. She believes in being a support for others.
LA'NITA WINFIELDCase Management Coordinator
La’Nita serves as the Case Manager Coordinator for the Elderly and Persons with Physical Disabilities (EPD) Waiver Case Management program. She is a Master-Level Social Worker with 10+ years of management experience. She is serving out her life mission as a “Change Agent”.
